Insurance & Financial Guidelines
Acura Dental Implants & Periodontics • Dr. Amer Atassi • Darien, IL
At Acura Dental Implants & Periodontics, we aim to remove as many financial surprises as possible so you can focus on achieving a healthy, confident smile. Below, you will find an overview of how we partner with dental insurance carriers and what that means for you as a patient of Dr. Amer Atassi.
We welcome all PPO plans
Our practice is an in-network provider with many Preferred Provider Organization (PPO) plans and will gladly file claims to any PPO carrier on your behalf. Whether your employer’s policy is national or regional, our insurance coordinators stay up-to-date on current reimbursement guidelines to help you receive every dollar of benefit you are entitled to.
- Primary claims: We file electronically the same day your treatment is completed.
- Secondary claims: If you carry dual coverage, we will either submit the secondary claim for you or supply the documentation you need to file yourself—whichever your plan requires.
Understanding the relationship: you, us, and the insurer
Remember, your insurance policy is a contract between you (and possibly your employer) and the insurance company—our office is not a party to that agreement. We will verify eligibility and obtain a pre-estimate of benefits before major procedures; however, this is not a guarantee of payment. The final responsibility for all fees rests with the patient or guarantor.
Our treatment recommendations are based solely on your clinical needs, not on the limitations of a particular plan. If an insurer reduces or denies reimbursement, you are still responsible for any remaining balance.
Key facts about dental insurance
- Insurance rarely covers 100 % of care
Dental benefits are designed to supplement—not replace—your personal investment in oral health. Typical PPO plans reimburse 50 %–80 % of an “allowable” fee after deductibles are met; the exact percentage depends on the premium paid for the policy and your employer’s contract.
- “Usual & Customary” (UCR) fees are determined by insurers, not dentists
If your Explanation of Benefits says our fee exceeded the “UCR,” it does not mean our charges are unreasonable. Each carrier creates its own proprietary fee schedule based on claim data it selects; numbers vary widely from company to company and often lag behind current market rates.
- Deductibles, annual maximums, and copayments affect out-of-pocket costs
Every plan sets its own yearly maximum benefit and may apply different deductibles to diagnostic, basic, or major services. Knowing where you stand within these limits—especially late in the calendar year—helps you and Dr. Atassi time treatment phases strategically.
